YSU Requestor Information

• The requester will need to complete the Vendor Information and Substitute IRS W-9 Form as shown above.

• This form can be sent to the vendor by email, fax, mail, or you can direct the vendor to the Vendor Information and Substitute IRS W-9 Form link above.

• This form must be completed and signed by the vendor to be accepted by Procurement Services.  The form should be sent directly to Procurement Services and once the information is reviewed, it will be entered into eCUBE.  

• The vendor can return the form to Procurement Services by email at newvendor AT ysu DOT edu, by fax at 330-941-3499, or standard mail: Procurement Services, Youngstown State University, One University Plaza, Youngstown, Ohio 44555.

• See above for links to the YSU Blanket Certificate of Exemption and Standard Credit Information forms.  If requested by the vendor, the department can complete the company name and date on the Blanket Certificate.

• You may send an email to newvendor AT ysu DOT edu with the new vendor name in the subject line.  This email will be matched up with the vendor’s form when they are returned to ensure that a University representative has requested this vendor.  When the vendor information has been entered, you will receive a reply to your email to let you know it is ready to use.

Vendor Information

• The vendor will need to complete the Vendor Information and Substitute IRS W-9 Form as shown above.

• Completed, signed forms must be forwarded to Procurement Services by email at newvendor AT ysu DOT edu, by fax at 330-941-3499, or standard mail: Procurement Services, Youngstown State University, One University Plaza, Youngstown, Ohio 44555.  Reminder: Only requested vendor forms will be considered and processed in our system.

• NOTE – YSU Policy requires all purchase commitments for goods and service to be made via an approved purchase order or through a charge to an authorized YSU Purchasing Card.  Vendors may experience significant delays in payment if they provide goods or services without proper/prior commitment.
