Enrollment Verification Request

Enrollment Verification Request (Letter):  If you have a form to be completed for proof of enrollment, we suggest the that you complete your portion of the form, file a request online, and attach the letter to the form before mailing it to the requesting agency (insurance company/loan institution, etc.).  Our office will send you the letter to the address you provide.  Unfortunately, we cannot fax an enrollment verification letter.  Please call the registration office at 330-941-3178, if you have any questions about this process.

Instructions:

1.  Go to the MyYSU portal >>> Student Records>>> Request Enrollment Verification

2.  Follow the promopts until you reach the point of entering an address.  Please skip down to "Issued To":

Place your name and address exactly like the example above; do not let your address exceed four lines.  The letter will be mailed to your address.

3.  Review the address you entered, and click the "Submit Request" button at the end of the form.
